VA Community Care Statement

Saber Tactical Performance & Rehab is proud to support veterans through the VA Community Care Network (CCN). This page explains how VA referrals work, what services are covered, and how veterans can request our clinic as their preferred provider.
1. Acceptance of VA Community Care Referrals
We accept referrals for physical therapy services through the VA Community Care Network.
Veterans may request Saber Tactical Performance & Rehab as their preferred provider during their VA appointment.
Once authorized, all physical therapy services are:
Covered by the VA
Scheduled directly with our clinic
Documented according to VA standards
Only the minimum necessary PHI required by the VA is included in HSRM submissions.
Submitted through HSRM
VA‑authorized physical therapy services are HIPAA‑covered. Performance Coaching and other non‑medical services are not subject to HIPAA and are stored separately.
There is no cost to the veteran for authorized PT visits.
2. What VA Referrals Cover
VA Community Care authorizations apply only to:
Physical Therapy (Medical Services)
Evaluation
Diagnosis
Treatment
Medical decision‑making
Rehabilitation
Progress documentation
Communication with VA providers
These services follow all VA and state practice requirements.
We use AI‑assisted documentation tools for medical note generation. These tools process only clinician‑entered information and do not access or use client media without consent.
3. What VA Referrals Do NOT Cover
The VA does not cover non‑medical services.
The following remain self‑pay:
Performance Coaching
Strength & conditioning
Tactical performance development
Movement optimization
Readiness training
Non‑medical assessments
Readiness scores and performance metrics are non‑medical and are not included in VA documentation.
Physical therapy services do not determine duty status, return‑to‑duty clearance, or occupational qualification.
Hybrid Sessions
Hybrid Sessions include both medical and non‑medical components.
Because VA authorizations apply only to physical therapy, Hybrid Sessions are not billable to the VA.
Hybrid Sessions are always self‑pay, even if part of the session includes physical therapy.
Dry Needling Add‑On
Dry needling is not covered unless explicitly authorized by the VA (rare).
In most cases, it remains a self‑pay add‑on.
4. How Veterans Can Request Saber Tactical
Veterans may request Saber Tactical Performance & Rehab during their VA appointment by stating:
“I would like my Community Care referral sent to Saber Tactical Performance & Rehab.”
The VA provider or Community Care coordinator will then:
Submit the referral
Generate an authorization
Send documentation through HSRM
Notify the veteran when approved
Eligibility, authorization, and visit limits are determined solely by the VA and may vary based on VA policy.
Once the authorization is active, we will contact the veteran to schedule.
5. Scheduling and Documentation
For authorized PT visits:
We schedule directly with the veteran
We follow VA documentation standards
We submit all required notes through HSRM
We communicate with VA providers as needed
Veterans do not need to bring paperwork unless instructed by the VA.
6. Adding Non‑Medical Services
Veterans may choose to add non‑medical services (Performance Coaching or Hybrid Sessions) at their own discretion.
These services:
Are optional
Are self‑pay
Do not affect VA‑authorized PT care
Are kept legally and operationally separate
This ensures full compliance with VA and state regulations.
