Welcome to Saber Tactical Performance & Rehab (“we,” “our,” “the clinic”). By accessing or using this website, booking services, or communicating with our clinic, you agree to the following Terms & Conditions. Please read them carefully.

1. Use of This Website

This website provides information about our services, policies, and operations. By using this site, you agree not to:

• Misuse or disrupt the website

• Attempt unauthorized access

• Use the site for unlawful purposes

• Copy or distribute content without permission

All content is for informational and educational purposes only.

Email and text communication may not be fully secure. By contacting us electronically, you consent to communication through these channels.

2. No Medical Advice

Information on this website is not a substitute for professional medical advice, diagnosis, or treatment.

Always consult a qualified healthcare provider for medical concerns.

Use of this site does not create a patient‑provider relationship.

3. Services Provided

Saber Tactical Performance & Rehab provides two distinct categories of services:

A. Physical Therapy (Medical Services)

Includes:

• Evaluation

• Diagnosis

• Treatment

• Medical decision‑making

• Rehabilitation

These services follow state practice laws and professional standards.

We use AI‑assisted documentation tools for medical note generation. These tools process only clinician‑entered information and do not access or use client media without consent.

B. Performance Coaching (Non‑Medical Services)

Includes:

• Strength training

• Conditioning

• Tactical performance development

• Movement optimization

• Readiness training

Performance Coaching is not physical therapy and does not include evaluation, diagnosis, or treatment.

Performance Coaching is a non‑medical service and is not subject to HIPAA. Physical Therapy is a HIPAA‑covered service. These records are stored separately and are not combined.

C. Hybrid Sessions

Hybrid Sessions include both medical and non‑medical components.

These services are billed and documented separately to maintain legal clarity.

Hybrid Sessions cannot be billed to the VA or insurance and are always self‑pay.

Readiness scores and performance metrics are non‑medical and are not used to determine medical readiness, duty status, or occupational qualification.

Physical therapy and performance services do not determine duty status, return‑to‑duty clearance, or occupational qualification.

4. VA Community Care Referrals

We accept VA referrals through the VA Community Care Network (CCN).

• VA documentation is handled according to VA Community Care and HSRM requirements, which may differ from standard HIPAA procedures.

• VA referrals apply only to physical therapy services

• Performance Coaching and Hybrid Sessions are not covered

• Veterans may request Saber Tactical as their preferred provider

• Authorized PT visits require no payment from the veteran

All VA documentation is submitted through HSRM according to VA standards.

5. Payment & Billing

Self‑Pay Services

• Payment is due at the time of service

• We do not bill private insurance

• Superbills are provided for out‑of‑network reimbursement

VA‑Authorized Services

• Covered by the VA

• No payment required for authorized PT visits

• Non‑authorized services remain self‑pay

Refunds

Refunds apply only to unused prepaid performance sessions.

Used sessions are deducted at the standard single‑session rate.

6. Cancellation Policy

A minimum of 24 hours’ notice is required to cancel or reschedule an appointment.

Late cancellations or no‑shows may result in a full session charge.

7. Client Responsibilities

Clients agree to:

• Provide accurate information

• Avoid sharing classified, sensitive, or mission‑related operational details during sessions.

• Disclose relevant medical history

• Follow safety instructions

• Use equipment responsibly

• Communicate changes in symptoms or health status

Clients may not film or photograph other individuals in the facility.

Failure to follow safety guidelines may result in discontinuation of services.

8. No Emergency Services

Saber Tactical Performance & Rehab does not provide emergency medical care.

We do not provide triage, crisis intervention, or urgent medical decision‑making.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

9. Intellectual Property

All content on this website — including text, graphics, logos, and materials — is the property of Saber Tactical Performance & Rehab and may not be reproduced without permission.

10. Limitation of Liability

By using this website or participating in services, you agree that the clinic, its providers, and its staff are not liable for:

• Website errors or downtime

• Misuse of information

• Injuries resulting from failure to follow instructions

• Unauthorized access to your data despite reasonable safeguards

Nothing in these Terms limits rights guaranteed by law.

11. Changes to Terms

We may update these Terms & Conditions at any time.

The updated version will be posted on this page with a revised effective date.
