How to Request Saber Tactical Through VA Community Care

Veterans can choose Saber Tactical Performance & Rehab as their preferred physical therapy provider through the VA Community Care Network (CCN). This page explains exactly how to request us and what to expect.
How to Request Saber Tactical
During your VA appointment, simply tell your provider:
“I would like my Community Care referral sent to Saber Tactical Performance & Rehab.”
Your VA provider or Community Care coordinator will:
Submit the referral
Generate an authorization
Send documentation through HSRM
Only the minimum necessary PHI required by the VA is included in HSRM submissions.
Notify you when approved
Eligibility, authorization, and visit limits are determined solely by the VA and may vary based on VA policy.
Once authorized, we contact you directly to schedule your evaluation.
What Services Are Covered?
VA‑authorized physical therapy services are HIPAA‑covered. Performance Coaching and other non‑medical services are not subject to HIPAA and are stored separately.
VA Community Care covers physical therapy services only, including:
Evaluation
Diagnosis
Treatment
Rehabilitation
Progress documentation
There is no cost to the veteran once authorized.
We use AI‑assisted documentation tools within SPRY for medical note generation. These tools process only clinician‑entered information and do not access or use client media without consent.
What Services Are Not Covered?
Performance Coaching, Hybrid Sessions, and other non‑medical services remain self‑pay.
Readiness scores and performance metrics are non‑medical and are not included in VA documentation.
Physical therapy services do not determine duty status, return‑to‑duty clearance, or occupational qualification.
Hybrid Sessions cannot be billed to the VA and are always self‑pay, even if part of the session includes physical therapy.
